Alive & Well Direct Primary Care memberships start as low as $163/ month for an employee, $205/mo for an employee + 1 family member, or $217/mo for an employee + 2 or more family members.
This includes unlimited access to our telemedicine line, scheduled visits to your place of work, house calls when appropriate, as well as major discounts on mental health and wellness services.
Employers who use Alive & Well typically pay a base rate for Alive & Well memberships and provide the option for employees to pay a small additional amount to include family members.
